Latest CMS Fee Schedule Updates & What They Mean for Clinics

The Centers for Medicare & Medicaid Services (CMS) has released its Calendar Year (CY) 2026 Physician Fee Schedule (PFS) Final Rule. For independent clinics, medical groups, and healthcare administrators, these changes represent a mixed bag of historic firsts, structural shifting of overhead allocations, and strict efficiency adjustments.

Failing to adapt your billing and operational strategies to these rules will result in immediate revenue leakage. Here is a breakdown of the structural shifts that went into effect on January 1, 2026, and how to protect your bottom line.

1. The Tale of Two Conversion Factors

For the first time in five years, CMS is implementing separate conversion factors based on your Quality Payment Program (QPP) status. Clinicians who meet the thresholds as a Qualifying Alternative Payment Model Participant (QP) will receive a higher baseline adjustment than those who do not.

Thanks to a temporary 2.5% increase stipulated by federal budget reconciliation legislation, both factors see a modest net bump compared to last year's base of $32.35.

Provider Classification

2026 Conversion Factor

Net Percentage Increase

Qualifying APM Participants (QPs)

$33.57

+3.77%

Non-QP Clinicians (MIPS / Fee-for-Service)

$33.40

+3.26%

The Revenue Impact: While a conversion factor increase is welcome news, the modest gains are largely offset by underlying structural methodology adjustments detailed below.

2. The Site-of-Service Shift: Community vs. Facility Settings

In an effort to promote site-neutrality and account for the growing number of physicians employed directly by hospitals, CMS has drastically overhauled its Indirect Practice Expense (PE) allocation methodology.

CMS is now funneling greater indirect cost recognition (rent, billing staff, scheduling overhead) directly to office-based settings rather than facility-based services.

  • Office/Community-Based Practices: Experiencing a net reimbursement increase of roughly 5% to 6% depending on the specialty.

  • Hospital/Facility-Based Practices: Facing a steep reimbursement decrease of roughly 7% to 11%.

If your providers split time between an ambulatory surgical center or hospital facility and an independent office, your billing team must meticulously audit place-of-service (POS) modifiers to ensure you are maximizing the community-based incentives.

3. The New -2.5% "Efficiency Adjustment"

One of the most controversial updates is the finalization of a negative 2.5% efficiency adjustment applied to the work Relative Value Units (RVUs) of non-time-based services. This blanket reduction hits diagnostic imaging, surgical procedures, and technical specialty codes hard.

What is Exempt?

To protect routine clinical workflows, CMS has excluded the following from the -2.5% adjustment:

  • Evaluation and Management (E/M) codes

  • Care management services

  • Behavioral health services

  • Brand-new codes introduced for 2026

  • Drug administration codes (exempt for 2026 only)

The Strategy: If your clinic relies heavily on procedural or imaging revenue, your overall productivity volume may remain steady while your actual collection per relative value unit drops. Optimizing your E/M leveling and fully capturing care management codes is vital to buffer this reduction.

4. Permanent Telehealth Definitiveness & Streamlining

The era of "provisional" telehealth billing is over. CMS has eliminated the distinction between provisional and permanent services on the Medicare Telehealth Services List. Going forward, the agency will review code additions based solely on a single metric: Can the service be effectively furnished via two-way audio-video communication?

Crucially for rural and institutional clinics:

  • No More Frequency Limits: CMS has permanently removed frequency limitations on subsequent inpatient visits, subsequent nursing facility visits, and critical care consultations delivered via telehealth.

  • Virtual Supervision is Permanent: The waiver defining "direct supervision" to include virtual presence via real-time audio/video technology has been adopted permanently.

5. New G-Codes for Advanced Primary Care Management

To reduce the administrative burden of traditional time-based behavioral health codes, CMS has introduced three new G-codes for Advanced Primary Care Management (APCM).

These codes eliminate strict stop-watch time tracking on behavioral health integration and Psychiatric Collaborative Care Model (CoCM) services, allowing auxiliary clinic staff to deliver care more fluidly under general supervision.

Action Items for Healthcare Administrators

To stay ahead of these updates, ensure your practice analytics and billing workflows are updated to address these three areas:

  1. Update Fee Schedules: Ensure your practice management system is updated with the localized 2026 dual conversion factors ($33.40 or $33.57) to ensure precise contractual adjustments and prevent underpayment tracking errors.

  2. Review Code Utilization Mix: Run a utilization report on your top 20 most frequently billed codes. Separate your E/M codes from your non-time-based procedural codes to forecast the exact financial impact of the -2.5% efficiency adjustment on your cash flow.

  3. Audit Behavioral Health Workflows: If you provide care management or behavioral health integration, transition your billing protocols to utilize the new, less restrictive APCM G-codes to maximize staff efficiency.
